  • Fill in sections 1-2, General Participant and Employment Information. (Note: Employer signature is required if you are not an AG credentialed minister.)
  • Select your investment option in Section 3:

    Track 1: MBA Fixed Income Option
    The MBA Income Fund pays a fixed rate that is declared quarterly with interest compounding monthly. It has had an attractive stable rate of return since inception over 50 years ago. Your invesment in this fund is used to build AG churches and ministry facilities across the country.

    Track 2: Individual Investment Options
    Individual funds that allow you to blend the MBA Income fund with a stock strategy.

    Target 3: Target-Date Strategies

    These strategies are based on your target date of retirement. These fully-screened strategies will automatically become more conservative as that date approaches.

    Track 4: Risk-Based Strategies
    Steward strategies offer a fully-screened index approach while the Russell strategies offer a non-screened active manager approach.
  • Designate your beneficiary(ies) in Section 4.
  • Sign and date the form in Section 5.

  • Complete the Payroll Deduction Form to request, from your employer, pre-tax salary reduction contributions to your 403(b) account or after-tax Roth 403(b) contributions.
