Collect all completed forms from each team member and keep on file at your church/organization. Completed and signed copies of the Assumption of Risk forms (adults age 18 and up) and Parental Consent forms (minors below the age of 18) for each team member must also be scanned and uploaded in their entirety at the beginning of the Mission Assure application. Forms should be provided via the application below or emailed to firstname.lastname@example.org. If you are unable to upload or email the forms please send us an email.
Each team coordinator is responsible for handling all team-related paperwork and submitting insurance payments. It is the responsibility of the applicant to accurately complete all required applications and forms and to ensure that they have been provided to the team coordinator for processing. Failure to do so will result in the prohibition of filing any claims. AG Financial Insurance Solutions will not be responsible or liable for any losses incurred by applicants failing to follow the prescribed procedures.
