How to Enroll
To enroll, submit the Enrollment Form and Bank Draft Form by fax at 417.447.7475 or mail them to AGFinancial Insurance, PO Box 10263, Springfield, MO 65808-0263.
Once we receive the completed forms, your insurance coverage will be added to the Standard Life Insurance plan. The bank draft will be processed through our billing company, Innovo Benefits Administration. We will be in touch either by phone or email concerning the premium, draft date, etc.
Rates and Maximums
The guaranteed amounts shown below are not subject to medical underwriting. If you would like coverage above these amounts, you will need to complete a medical history form to prove insurability.
Credentialed Minister
Under age 85: minimum $10,000 and maximum $50,000, in $10,000 increments
If age 65 – 84, there will be a 35% reduction in this amount when enrolled.
Spouse (Dependent Coverage)
Under age 85: minimum $5,000 and maximum $10,000
If age 65 – 84, there will be a 35% reduction in this amount when enrolled. The reduced amount will be no lower than $5,000.
Child (Dependent Coverage)
2,500, $5,000, $7,500 or $10,000 (all guaranteed)
Child(ren) are eligible through age 18 or through age 24 if attending an accredited educational institution full-time.
Spouse and Child Coverage cannot exceed 100% of the amount of coverage elected by you as the credentialed minister.
Rates and Coverage Changes
All rates are 5 year step-rated, meaning rates increase in 5 year increments (ages 25, 30, 35, 40, 45, 50, and so on). Insurance coverage reduces by 35% at age 65 and terminates at age 85. These changes all occur the January following the birth date of the insured.

Higher Coverage Amounts
If you would like to apply for a higher amount of coverage than what is listed above, you will need to complete a medical history form to prove insurability. Coverage over the above guaranteed amounts will not be in effect until approved by Standard Insurance Company. Higher coverage limits are as follows:
- MINISTER – UP TO $500,000 AVAILABLE IN $10,000 INCREMENTS
- SPOUSE – UP TO $250,000 AVAILABLE IN $5,000 INCREMENTS
*The coverage amount for the spouse cannot exceed 100 percent of the newly credentialed coverage.
